  Dear Sir/ Madam,

  Olsen's Grain and Purina Mills Inc. would like to invite you to 
 participate as an exhibitor during our ARIZONA EQUIFEST®.  
 This is a Two-day event held at the Prescott Rodeo Grounds, in 
 downtown Prescott.  
 The Equifest gates will open on Friday at 4:00 p.m. and close at 
 8:00 p.m. on Friday September 21st, 2012. 
 Gates will re-open on Saturday Morning  at 8:30 a.m. and  close 4:00 p.m.  
 The Equifest® is a free to the public event brought to the community by
 our great sponsors. 

  We are planning extensive advertising for this event and  expect
 a large turnout.
 Olsen's Grain hopes with the exhibitors' help, to bring new information 
 and products to our community.
 We anticipate our Arizona Equifest® to be very successful by making it 
 a weekend event for families to enjoy with more education and great 
 entertainment for all who attend.

  The following schedule for set up times is: 10:00 a.m - 3.00 p.m. on 
 Friday Sept. 21st. 
 All booth spaces are a 10 x 10 space and 2 chairs will be provided with
 your booth purchase.
 You can also download the application from our website at
  www.olsensgrain.com. 
 If you would like to participate in this great community event please 
 complete the exhibitor application and contract and return it along 
 with your payment to Olsen's Grain Attention:
 Angel at P.O. Box 427 Chino Valley, AZ. 86323.

  Please be aware that the Prescott Rodeo Grounds will use both under 
 roof and outside areas for our exhibitors.  
 Booth assignments will be made on a first come first served basis and
 you will not be able to state your preference on the application. 
 We plan on making sure there is a steady traffic flow through the 
 outside locations as well as the grandstands.

accompanied by the fees due.  By executing this application and contract, the exhibitor 
agrees to abide by all terms, rules and regulations hereof governing the 2012 Arizona EquiFest 
as described on this application. Please read all rules below. 
Such rules are hereby expressly incorporated herein by reference and agreed to by exhibitor. 
There are no refunds. Booth spaces will be issued on a first come first basis. 
EquiFest coordinators reserve the right to change booth arrangement or alter booth assignments 
without prior notification to the exhibitor. EquiFest utilizes both outdoor and indoor venues 
for exhibitors and proper shade or covering is vendor's responsibility.
